(GAIP) - File a Claim

If you go to a preferred provider you do not need to file a claim. Simply present your ID card at the time of service. Your ID card is mailed to you by the plan administrator, Welfare and Pension Administration Service (WPAS) when you enroll on line. You can also print a generic ID card (PDF).

Please remember that the card, whether you print it out here or receive one from WPAS, is not proof of coverage. Coverage can be verified through WPAS (see contact information below) after the 10th day of the first month of the coverage period, and by Benefits & Work/Life before that time.

If you go to a provider who is not part of the WPAS network, you will have to file a claim for reimbursement. to file a claim, fill out a claim form.

to fill out the claim form, you will need the plan number. The 2007-2008 plan number is 2007-1464-2. You will also need your Social Security Number (SSN). Appointees who do not yet have a SSN can use their employee identification number.

Mail your claim, with a copy of the bill in question, and any related communication to :
