Eligibility to participate in UW-sponsored medical, dental, life and long term disability insurance programs depends on your job class title, employment percentage, and the length of your intended employment.
Review eligibility periodically. Life events such as adding a spouse or qualified domestic partner; birth or adoption of a child; or death of a family member can impact insurance eligibility. You are responsible for notifying Benefits & Work/Life of any changes in your family circumstances. Prompt enrollment or removal of a dependent ensures access to benefits when needed, and that unnecessary benefits will be stopped appropriately.
Use the Benefits Eligibility Chart below to determine if your position is benefits-eligible, when you are able to apply for coverage, and when your coverage begins.

*Important: Employees must complete the enrollment form required to activate coverages.
** Temporary employees whose department indicated on required employment forms that the employee would work 6 or more consecutive months at half time or greater, are eligible to apply for benefits from the beginning of their appointment.
