Long-term disability (LTD) replaces a portion of your income if you are unable to work due to illness or injury as defined as below:
Residents and Senior Fellows enrolled in the Schools of Medicine, Dentistry, Nursing and Pharmacy at the University of Washington are eligible for this plan.
Premiums are $18 per month; the premium is paid through direct payroll deduction.

You must apply within the first 31 days of entry into your residency or senior fellowship and will not need to provide evidence of good health. Your online enrollment first goes directly to the plan administrator, Professional Benefit Consultants, for processing, then it will be transmitted to Benefits & Work/Life to begin payroll deduction. The coverage will be effective the first day of the following month. Please be sure to print a copy of the online enrollment form for your records. If you do not receive confirmation of your application within five business days, please call Parker, Smith & Feek at 425-709-3600 and ask for the Benefits Department.

Claims are filed by contacting Parker, Smith & Feek at 425-709-3600. They will notify Benefits & Work/Life so that all of your UW benefits can be coordinated.
Benefits end when your Resident or Senior Fellowship ends. If you are continuing employment with the UW after your Resident or Senior Fellowship then you must contact Benefits & Work/Life to ask for your deductions to stop. You may send an email to benefits@u.washington.edu to notify Benefits & Work/Life of your appointment change.
To continue LTD insurance coverage when your Residency or Senior Fellowship ends, please contact Parker, Smith & Feek at 425-709-3600 and ask for the Benefits Department.
