UNDERSTAND

Long-term disability (LTD) replaces a portion of your income if you are unable to work due to illness or injury as defined as below:

• Within the first 60 months of diagnosis – You are receiving care from a physician and are unable to perform the substantial and material duties of your Residency or Senior Fellowship.
• After first 60 months of diagnosis - You are receiving care from a physician and are unable to perform the substantial and material duties of your Residency or Senior Fellowship, and you are unable to work in the capacity of a Doctor of Medicine based on education, training and experience.
• Seropositive HIV - Paid as a Total Disability Claim.
• AIDS Diagnosis - Paid as a Total & Permanent Disability Claim.

Eligibility

Residents and Senior Fellows enrolled in the Schools of Medicine, Dentistry, Nursing and Pharmacy at the University of Washington are eligible for this plan.

Premiums

Premiums are $18 per month; the premium is paid through direct payroll deduction.

Monthly Benefit

• Monthly Benefit – Total Disability: $2,500 flat per month during all years of residency. Total & Permanent Disability: $5,000 monthly for first 12 months. At 12 months the prognosis must be reconfirmed. After the prognosis is reconfirmed there is a $5000 monthly payment during all years of Residency or Fellowship, plus a one-time lump sum payment of $5,000.
• Benefit Offsets - Totally Disabled: If base plus other sources of income exceed $5,000, then benefits would be offset above this level. Total & Permanent Disability: If base plus other sources of income exceed $10,000, then benefits would be offset above this level.
• If you are totally disabled any income you receive in excess of $5000 is deducted from the benefit payment. If you are totally and permanently disabled any income you receive in excess of $10,000 is deducted from the benefit payment.
• Loan Payoff Benefit - Repayment of up to $175,000 of eligible debt.

Enroll

You must apply within the first 31 days of entry into your residency or senior fellowship and will not need to provide evidence of good health. Your online enrollment first goes directly to the plan administrator, Professional Benefit Consultants, for processing, then it will be transmitted to Benefits & Work/Life to begin payroll deduction. The coverage will be effective the first day of the following month. Please be sure to print a copy of the online enrollment form for your records. If you do not receive confirmation of your application within five business days, please call Parker, Smith & Feek at 425-709-3600 and ask for the Benefits Department.

File a Claim

Claims are filed by contacting Parker, Smith & Feek at 425-709-3600. They will notify Benefits & Work/Life so that all of your UW benefits can be coordinated.

Benefits End

Benefits end when your Resident or Senior Fellowship ends. If you are continuing employment with the UW after your Resident or Senior Fellowship then you must contact Benefits & Work/Life to ask for your deductions to stop. You may send an email to benefits@u.washington.edu to notify Benefits & Work/Life of your appointment change.
