2009 Premium Rates
for Life, Accidental Death & Dismemberment Insurance

UW Paid Life Insurance

The UW pays for $25,000 of life insurance on each eligible employee. If you go on leave or layoff status and wish to continue optional life insurance on yourself, you will be required to pay the UW's contribution (see “Part A Basic,” below) for this insurance during that period.

Part A Basic

Employees
$5.07 per month (employer-paid)

Part B Basic (for Dependents)

An employee pays $0.50 per family, regardless of the number of dependents.

Part B Supplemental, Part C Optional, and Part D Supplemental Insurance

Your premium rate changes to the next higher rate as you reach each new age bracket.
